I created a form for customers to submit data via email. Everytime you go to
submit the info on the form - by hitting the submit button - you get me logon
and password showing up on the screen. This only happens once - if you filled
the form out again and hit submit it doesn't show up again. Any clue to what
is happening?

probably some kind of funky permissions on the file or folder that the form
data is getting saved to.
do you have some kind of "control panel" your host gave you for the domain
in question?
