forms problem

Hi,
When I moved my pst file from outlook 2000 to outlook xp one of my custom
form cannot be opened. The exact message I get is

"The custom form could not be opened. Outlook will use an Outlook form
instead. The form required to view the message cannot be displayed. Contact
your Administrator"

Since I have used some custom fields in my form there is now data that I
cannot see. What can be the problem and how can I fix it?

What steps did you follow to "move" the .pst? Is the same .pst the default
information store? Where was the form published?
