Forms have stopped working

We have a number of subsites that use small forms.

These forms use the SaveResults bot to save information into the private
folder. When submitting the information now does not get saved to the file.
No errors that I can see. FPSE 2002.

Page extensions tried: .htm .html .asp .shtml .shtm

htm html shtml shtm are mapped to ssinc.dll
asp mapped to asp.dll

Is there something I'm missing here??
