Form with preset tasks

Hi I wonder if anyone can help. I want to create a form which has a main header date, user etc....instructions for each tasks required, yes/no box and a comments box... There will be 20 plus instruction lines as above and the user can print them off

Problem is I can't think if the best way to lay it out so that I can capture which tasks are complete or not and the comments in a report.

Any ideas?
