Print it and use a pencil or pen, or even a typewriter. Otherwise, use
software designed to do what you want. Forms software comes in the PaperPort
utility that came with my scanner. Word is not designed to be used for this
purpose.
If you are trying to convert a form to electronic format for use in Word on
a longterm basis, look into what Word calls an "online form." Check this in
