File As Field Not showing in Contacts

Hi.

I use Outlook 2003 and for some reason the "file as" field is not appearing
for new contacts that I enter. It only shows a pale gray highlighted field
with no information in it. My other contacts are still ok and alphabetized
properly.

Can anyone help me restore this?

What settings did you designate for the File As field?
What message store are you using--Exchange mailbox or PST?
