Email Account Not Listed

I'm connected to an exchange server mailbox that has been removed from the
server. It's still listed under All Mail Folders along with my main mailbox
and my archive folders. When I right-click and select "Close" I get a dialog
that says I have to go to Tools > E-Mail Accounts and Remove it from there,
but when I go there it isn't listed. How can I remove the account?

Try right clicking on "outlook today" or "mailbox - <username>". Select
Properties > Advanced button > Advanced Tab. Section at the top is where
additional Exchange mailbox can be added/removed from your Exchange mailbox
profile.
