While running a 2k domain, account become locked after 6
failed attempts. Even after six we still have some who
continue to lock themselves out. We also have our policy
to auto-unlock after 30 minutes. Problem is that it never
resets, or is this not how it's supposed to work?

Depending on the policy, if a lockout is engaged, the account will
re-lockout if another wrong password is attempted.
