delete recurring organizational chart

My son accidentally inserted an organizational chart into a Word document.
Now everytime I open a new document, the chart is there & I have to delete
it. It also shows up in any email I forward. I can manually delete it but I
have to do it everytime I open a new document or forward an email. I have
tried every help question I can think of and nothing fits. How can I get rid
of this chart permanently?
