Cutom toolbar changes Word

When I customise the buttons on various toolbars the changes are not
re-applied when I shut and then re-open Word. What's odd id that where I
have activated a new toolbar (eg: Picture toolbar) that does re-appear. The
problem only exists where I have added buttons to a toolbar.

Note that the toolbar changes must be saved in normal.dot or in an add-in if
you want them to be available at all times.
