Custom Form in Outlook Web Access 2003

I've created a custom form in Outlook 2003 for a Public Folder based on the
standard Task form. I need to know how to make this form available in
Outlook Web Access 2003.

You would need to build and deploy a comparable form from scratch using Exchange web forms techniques, which are totally different from Outlook form techniques. Docs start at http://msdn.microsoft.com/exchange/ with discussions in the microsoft.public.exchange.development newsgroup

There are several articles regarding the "Microsoft Outlook HTML Form
Converter", but this application doesn't seem to be available for Exchange
2003. Has Microsoft discontinued this application? If so are other tools
available to replace it or is there no convenient way to customize Outlook
forms for OWA.

That application was for Exchange 5.x only, which used a completely different web forms technique. There is no conversion tool for later versions.
