Custom Form Error when Opening Contact

I'm using Outlook 2007 and I've created a Custom Form for my Contacts (I had
been using just fine for months and then last month the error ocurred and
continues every day at RANDOM situations).

I've received the following messages at various times:
"The custom form cannot be opened. Outlook will us an Outlook form instead."
The form required to view this message cannot be displayed. Contact your
administrator."
"Outlook cannot complete the save due to a file permission error.
(C:\Users\AppData\...~WRD0004.docx)"

I've tried clearing the "form cache" in Outlook and this did NOT correct the
problem. I've also tried deleting the "FRMCACHE.DAT" file under Forms folder
in Local\Microsoft and this did NOT correct the problem.

Each time I receive the errors above, Outlook opens the Contact but in the
Original/OLD Outlook default form. In order to see my form, I have to shut
down Outlook and restart.

Please advise how I can correct the problem so I can ALWAYS use my custom
Contact form and not receive any type of error.

Where is the form published? Is it possible that it's published to more than one forms library?

Have you tried deleting the entire contents of the Forms folder?
