CSVDE Import into ADS

I am trying to do a bulk import of over 300 user accounts
into ADS (on a win2K box) with Exchange 2000. I want to
create both user accounts and exchange mailboxes using
the CSVDE import utility.

Server Information is as follows:

name: vha-svr-01
ADS name space: vhaottawa.ca
Netbios domain name: vha-ottawa

When using the CSVDE import utility I am receiving the
following error.

Connecting to "(null)"
Logging in as current user using SSPI
Importing directory from file "c:\import.csv"
Loading entries.
Add error on line 2: Constraint Violation
The server side error is "A required attribute is
missing."

An error has occurred in the program
