Where can I get a dispute letter that I can send to the Credit Reporting
Agencies that will prompt them to investigate inaccurate information on my
credit file(s)?

databases.

Since your situation is unique, I doubt you will find a form letter for
this. Simply write them and tell them what you are disputing and why. Ask
that they investigate it or remove it from your credit history. If the
credit reporter does not respond to the credit bureau within a certain time,
the item is typically removed.
