Creating headers in a template - form

I want to create a header in a form (template) that picks up information
(data) from two fields within the form. In my case this is a client record
that I want the Client's name and our assigned client ID number to post as a
righthand header on every page except page 1. My form is protected. I think
I made this work about 5 years ago but unfortunately I've forgottn how and do
not have the file/form I created then.

I think I answered this question somewhere else. In the Header, insert
cross reference fields to the bookmark names assigned to those formfields
and set the calculate on exit property for those formfields so that the
fields in the document are updated when the data has been entered into them.
