Creating custom message templates / forms

Hello, I want to create a form for e-mails that will allow me to have a
blank to fill in for a model number (for example) and a few radio buttons or
check boxes to click in order to send an electronic request on items with a
standard format. Any ideas on how to do this? Thanks!

To send to whom? Unless you're sending these around within an organization using Exchange as the mail server AND you have permission to publish forms to the Organizational Forms library, Outlook custom forms are not a viable solution for this scenario.

We will be going to an exchange server with web access only in about a week.
Is this a viable solution at that point if users are accessing via the web?
If no, what might be a good workaround. Thanks for your help.

Not at all. Outlook Web Access cannot use forms created with Outlook, and server-based web forms that work with Exchange present various hurdles. You might want to look into Microsoft InfoPath and applications from other companies that deal with web-based forms.
