create login form with username and password

hi,

i have data which is contain lots of people details....and i would like to create a form....password and usernames are alrdy in the table...i just want to design the form and when i press the button i want to see the user record which is enterd in the username box..can anyone help me wt this plzzzz...i need to use macro for this...its part of my college work...
