create a new form in Microsoft Outlook or edit an existing form

I am attempting to create a new form to be used with Calendar in Microsoft
Outlook. This new form will serve the same purpose as the standard
appointment form but will have different fields for information that is
needed for a particular type of appointment.

Yes. My particular question is how do I either create a new form or modify
the existing appointment form? No matter what I do I am unable to open the
form in design mode.

So what precisely happens when you choose Tools | Forms | Design a Form and choose the appointment form?

The form opens but it is not in design mode. All of the design icons are
visible but they are grey and cannot be used.

Did you read the web page I suggested earlier at http://www.outlookcode.com/d/appointmentform.htm? If you had read it , you'd know that the first page of the appointment form cannot be customized.

I appoligize for not having reviewed the web pages you sent. I just have not
had the time today. If I cannot modify the Appointment form then I would be
just as happy creating a new form that we could use in it's place. Can you
help me with how to acomplish that task?

That's not what I said. You cannot modify the *first* page of the appointment form. You can modify 5 other custom pages that are available. The other page I cited -- http://www.outlookcode.com/d/forms.htm -- has links to walkthroughs and other resources to show you how.

Sue,

I really appreciate you help with this task. I was able to modify P.2 of
the Appoiintment form and get it saved, and it seems to be working well. I
have one additional question. Is there a way to make the newly published
form the default appointment form in Calendar? In other words have it come
up instead of the standard appointment form when the new button is pressed.

Again, thank you both for your help with this task. I read between the lines
of your message that you think that I should have found this answer by
looking through all of the pages that were mentioned in earlier messages. I
appoligize for not having done that but I am not a 'developer' and I have a
limited amount of time to spend on tasks like this between doing my assigned
job, even though once I am successful the result will make my job and others
at our companies' jobs easier.
