Cover Page Fields

Is there a list somewhere of *all* of the fields that can be specified in a
personal cover page for automatic fill-in; e.g., {Recipient's Name},
{Subject}, {Note}, etc.? This would seem to be an obvious inclusion in the
help file, but no...

I should have noted that I'm looking for fields beyond those offered by the
"Insert" pull-down menu. My specific need at the moment is something to
pull in the sender's e-mail address, which is specified in the fax
configuration, but seems not to be used anywhere.
