Completed details on form don't show on reply

I'm have created a form for a training feedback questionnaire for internal
staff who are all using outlook wtihin the same exchange org.
I have started the form by doing tools | Forms | Design Form and I'm
creating one within the Personal Forms Library at the moment. When
completing the questionnaire and replying, I have got the form to appear on
reply, but any of the answers when completed don't show on the reply. And
other users i send this to, are unable to reply, even though they can
initially open the form.

Message forms need to be published to the Organizational Forms library or to
every user's Personal Forms library; see
http://www.outlookcode.com/article.aspx?id=61.

We could use more detail on what process the user goes through to "reply."
Pay attention in particular to whether the data entry controls are bound to
actual Outlook properties.

In general, Outlook forms do not make good survey tools. InfoPath or the
combination of Access 2007 and Outlook 2007 would be better.
