Change what form existing items open with

I have published a custom contact form that I use for creating all new
contacts. However, I am having trouble making Outlook use that form to open
existing items (that is, contacts that were created before I started using
the custom form).

I tried publishing the custom form as the default form, with no luck.

Class of pre-existing contacts are:

Maybe you can export your existing contacts to Excel. Then import them back
to Outlook using the Mapping feature to get the information on to your new
form. Not that easy, but I think it should work. Now can you help me to
create a custom contact form?
