Cant rename Index.htm as .html : because "index_files" already ex

While developing, I have been using .htm, but for live site I need to use
..html extension.

When I try to change Index.htm to .html (via the folders view as exampled in
the Help text) ... I eventually get the message - can't rename
..../index_files to .../index_files because the file (or folder??) already
exists.

I tried deleting the index_files folder (via Explorer), but then FP said it
couldn't rename because index_files doesn't exist.

I seem to be in FP's version of Catch 22.

I noticed that when I publish to the live site, index does actually get
named index.html. What controls this?
Is it connected, or the reason for the issue above?

The folder index_files is super hidden in FrontPage and hidden in
Windows Explorer.
In FrontPage, the only way to change the extension on a file that has an
associated *_files folder, is to rename the file twice.
First, rename index.htm to default.htm, allow all links to update. Then
rename again to index.html and allow all links to update.

When you publish to a server FrontPage automatically renames the Home
page to name and extension required by the server.

The folder index_files is created when you use VML graphics (WordArt,
ShapeArt, Text-Boxes) in the page, or use an Office application (Word,
Excel, PowerPoint, Publisher) to create the page.
