Can't Create A New Connection

On the "New Connection Wizard" .... internet connection,
after selecting the "Set up my connection manually", the
next page' "Connect using a broadband connection that
requires a user name and password." is grayed/disabled -
only the "Connect using a broadband connection is always
on" option is available for selection.

Please Help..
