Cannot run the FrontPage Server Extensions

After trying to add a form to a page footer, I received the following error
message:
Cannot run the FrontPage Server Extensions' Smart HTML interpreter on this
non-HTML page:
"http://www.cloud9coaching.com/percentage/start_gallery_17.htm

Also, above the form on the published page is [Front Page Save Results
Component]

I tried removing the email option from save results, but it didn't fix it.
How to fix? TIA

If you want to use the FP form handler, then place the form on it's own
page, not in a shared border, etc.
