Away to keep tables up to date- any idea

I five (Client)locations that use my database. The
problem I am having is that if I make a change to
backend "tables". Like add new tables, relationships,
fields ect....

When I go to give my client the updated version of the
frontend I have to edit there backend (keep there data
correct)to resemble the new one I created this takes a
very long time to do. Also some clients have one version
others have another, this is hard to keep track of ????

any software or code out there to do this
