Adding custom form data into the Message Body

Hello all,

I'm quite new to this, and (remarkably) even a Macintosh user myself.

But ofcourse my company uses Windows, Outlook and Exchange, and because I have a fascination with programming/UI and efficiency I've delved into customizing outlook to meet some coworkers needs.

I've created a custom form in outlook for them to take phone messages, with fields like name, phonenumber, mail, etc. It works great and I've even figured out how to put it in the toolbar and adding an electronic signature to the macro so it doesn't compromise security.

It works great internally, but ofcourse not on a Macintosh, and also not on Web Access.

So this is what I'd like to see in the recieved message:
Subject field: (data from the name field) Left a message, he can be reached at (data from the phonenumber field)

Or alternatively:
Message field:
Name: (data from the name field)
Number: (data from the phonenumber field)
etc.

I can't find this scenario worked out anywhere on the internet, although to me it seems to be a very usefull option.
Is there a way to do this?
