I am attempting unsuccessfully to create a simple yes/no questionannaire for
file reviews. The each review will consist of:
Employee
District
Date reviewed
Customer name
Customer id number
10 questions
yes/no answers
after creating the table, I need to be create reports stating the number of
yes/no by employee, district both by quarter and year. I have tried several
different databases. I am a novice so please be gentle. Thank in advance
