Word Input Dialogue boxes

I am trying to create a word template where Dialogue boxes appear when a user
opens the template that ask questions, e.g. Enter Name, Enter Address and so
forth. Once the questions cycle through and the user reaches the end of
them, the information is then presented at various points in the document.
For example, if I created an application form, the form details would already
be contained within the document, but the user would need to ask questions
that open up in a dialogue box before the fields were populated. I have seen
this before but have no idea how to duplicate it.

The "dialog boxes" you saw are called userforms. There are tutorials
on creating simple userforms at
