Word & Excel Template Combined

I produce survey reports using the same word template. I also have an excel
template which I use for calculations. How do I make a template which
combines the two?

Can you be more specific about what you have in mind?

If you mean, you'd want a new file from each of these templates, linked to one
another, then there's no way to realize this without using automation
(programmed macros).
