Why doesn't added words work correctly in spellcheck?

I'm using Office 2000 and am a medical transcriptionist. When I add words to
the spellcheck and then use that word again, when it is checked the
dictionary shows the word divided rather than in one complete word. This
has happened to numerous words that I have added.

Try asking this in the word.spelling.grammar newsgroup, and please give some
examples of terms this happens to.
