Why do the list numbers disappear?

Sometimes, when typing I will create a numbered list that later disappears as
in the list is no longer numbered or it stops being sequential so that
instead of 1,2,3,4 I'll get 1,1,1,2 or all 1s. Anyone else have this problem
and know how to fix it?

I'm guessing that you are using Outline Numbering.

Have you had the same problem? Is there a way to fix it?
