Why did I get the additional word "SPAM" in the subject line?

A sender sent me a message. In the subject line this was added to the subject
line. "***SPAM***
This was from a trusted person. I confirmed with this person that it was not
intentional.
 
This is not a native Outlook function. Either your ISP does this or you've
got a third party tool installed that does spamfiltering and tags the
messages.

That's the default spam tag for Qmail-Scanner. More than likely
whoever hosts your email tags all spam with that ***SPAM*** line. Call
them and see if they can add that sender to their whitelist.
