That page may have a couple screen shots but doesn't tell me how to make
them. I was hoping for more of a reference/how-to resource.

There aren't many how to pages on folder home pages but you can load the
html sample pages (there are live pages there that the screen shots were
taken from) with the controls and read the source code. FWIW, you can use
any HTML page as a folder home page - it all depends on what you want.
