using the "send to" dialogue box

How do I add destinations to the "send to" dialogue box--e.g., adding a new
folder to the dialogue box to which I can send a file or document?

Perhaps I'm missing your point, but as the question reads, you don't...
That's what Save As (File Menu) is for. Use the 'New Folder' button in that
dialog box when necessary if you need to create a new folder on the fly. Just
make sure to navigate to where you want the new folder created _first_,
otherwise you will have to move it later.

Depending on what you mean:

1. Destinations cannot be added to Word's File | Send To menu.

2. If you want to add folders to the shortcut menu you get in the File Open
dialog when you right-click and choose Send To, you need to create a
shortcut to the desired folder and put it in the C:\Documents and
Settings\<username>\SENDTO folder.
