Using Headers with Form Field Document

I have a 5 page document that is form fields for opening new accounts
Is it possible to add form fields within the header to placing the users
name/date and account data on the top of each page automatically.
Any suggestions on how to do this would be greatly appreciated.
 
No. When your document is protected for forms headers and footers are not
available. Look into the Ref and StyleRef fields. You can also insert the
username and date using automatic fields rather than form fields. What you
are talking about is what Word calls an "online form." For more about online
