usbohci.sys STOP 0x000000D1 problem

Whenever I plug a usb device into a usb port windows xp
automatically shows the a blue STOP screen saying
something is wrong with usbohci.sys driver, what should I
do?

I get the same thing with my Geforce 4 card and crash

when i run my antivrius... They have said it's a driver
problem . Haha i have used every driver i can find with
the same result . The blue screen
