Unlock computer box

My dialog box shows my user name twice, once with the domain name and the
other right after that in parentheses and as far as I know no one else seems
to have this problem...Is there any way I can get the second name out of that
box?

Check if two users exist in the Control Panel > User accounts and if
so, delete the unwanted account.

Sean,
Make certain that before you remove an account, that you have checked on & moved any files/documents in it's My Documents folders.
