Two Guys in a Truck

"Two guys in a truck." One is on the job, the other is riding along with a
check list evaluating how the other guy is doing his job.

I cannot find a template with this common scenario. In Excel we have a form
that needs to be put into a database that will be used on a laptop out in the
field.

The majority of the form will be questions (about 100 questions) with three
columns for the answer, e.g.,

1. DID THE TECH CALL BEFORE THE APPOINTMENT TO CONFIRM DATE/TIME?
Yes No N/A

2. DID TECH VERIFY THE ORDER OR TROUBLE REPORTED WITH THE CUSTOMER?
Yes No N/A

The other section has the same with a 3-column scernario of Pass / Fail / NA
ELECTRICAL HAZARD PROT Pass / Fail / NA
HARDHAT Pass / Fail / NA

Anyone aware of a template available with this scerario? Or any
how-to-approach-this suggestions?
