tool bar problem

I am running windows XP home edititon with service pack 2 and am having the
following problem.

I have open say 1 occurence of any software package (arguments sake it could
be ms hearts or it could be explorer doesn't matter) instead of appearing
once in the toolbar it appears twice, if I unlock the toolbar it appears in 2
different sections or zones. The number of applications open is not important
each application will be repeated.

How do i eliminate it from appearing twice, I would like it to appear once
for ease of use and clarity.

Any help would be greatly appreciated....

Are you double clicking a single click icon, and for some programs
that allows you to actually start running 2 sessions of the same
program (i.e the 2 programs you mention are classic examples)?

Test : open a program that has 2 icons in the Task Bar. Make a change
to the one currently showing on the screen and then click on the other
icon to switch. Is the other program the same, or in its original
startup form?
