First, "automatically update" should seldom, if ever, be checked.
Is your TITLE BOLD style a character style? It sounds like it. Anyway, the
style applied to a TOC entry is defined by the TOC styles, not by the style
used in the source text. For instance, I normally want my chapter headings
in 18-point bold centered text. I don't want that formatting applied to my
table of contents!
When I type in a style that has all capitals, I still use appropriate
capitalization in my typing if I want to pick up what I type elsewhere using
a field. A TOC is using a field to pick up information typed elsewhere.
You might want to change your TITLE BOLD style to use small caps instead of
all caps. It looks classier and you'll be able to see if you are
capitalizing words you want to be capitalized in your TOC.
I'm sure this isn't what you wanted to hear, but hope it helps.
