The word "index" doesn't index correctly

I'm indexing a text about stock market indexes, and I need the many references to indexes to appear in the Index at the end of the book. But when I create subenteries, most (but not all) list seperatley. Maybe Word 2002 treats the word "index" specially? Yes, I checked for consistent capitilization and no extra blank spaces

Make sure there is absolutely no difference between the XE fields. For
example, make sure that you don't have an extra space after "Index" in one
of them or different capitalization or any other subtle difference. That
said, however, the first index I ever made did the same sort of thing. The
book was a family history of a family named "Link," and I had the same
problem with the name "Link" (and eventually some others as well); in my
case, it worked fine up to a certain page and then got balky.
