"The operation has failed"

We have Outlook 2000. On one of our machines, when you try to open and Excel
or Adobe attachments, frequently, the error message "The operation has
failed" appears. If you close Outlook and reopen it, the attachment will
open without problem. What is causing this and how can I fix it?

We have the same issue:

It happens because Outlook is opening more than once in the background. You
can kill it through Task Manager and end the process.

I am not sure why it opens twice..is Outlook in the Startup menu and then it
gets launched manually?
 
Outlook is not in the startup menu. It is only opened manually. We do use
the Microsoft shortcut bar, though, and the user has two different icons on
his desktop. One shortcut is on the quick launch bar and one shortcut is on
the actual desktop.
