Synchronising Outlook instances across internet?

I use Outlook both at home and work,
and currently synchronise contacts,
tasks, etc, between both by synchronising
with my iPAQ. However, is there an option
of keeping both in sync across the
internet? (Both machines are usually
connected 24/7 by broadband). If so, would this
also enable me to synchronise my journals
(the iPAQ doesn't current offer this
facility).
