You're registration number is useless in determining this and you should
never post this type of information.
That said, open Control Panel, open System, go to the Hardware tab, click
Device Manager, click the plus sign next to "Sound Video and Game
controllers, this may reveal the model. If not, in Control Panel, go to
Sound and Audio Devices, you may see the model number listed there.
Also, check the documentation that came with your computer, it may have some
information, you might also contact your Laptop manufacturer as they may be
able to give you the correct information.
Also, note, you may have an OEM card in which case, only the Laptop
manufacturer can give you the information as generally OEM cards don't
conform to retail model numbers and often the drivers are located in a
different area or are different from the standard drivers. You might also
check the Polk Audio web site as they may have more information that will
help you properly identify your card.
