Setting up a form

I have set up a form where I have typed in NAME: than tabbed over and typed
DEPARTMENT - both of these words are followed by drawn lines so the name can
be typed on the line when completed as well as the department. I have
limited the number in the text field, however, when I lock the document
because I have tabs between NAME and DEPARTMENT the word Department scrolls
to the next line when typing in the name. I want the word DEPARTMENT to stay
where I have it on the form. How do I do this?
 
What you are talking about is what Word calls an "online form." Check this

You would put your fields and prompts into table cells or frames that are
fixed in size.
