Word is not set up to do this well. If you are only going to be using the
form once or twice, you will do a lot of work for little benefit. PaperPort
comes with a utility to let you fill in scanned forms.
If you are going to use such a form over and over it may still be easier to
recreate it in Word than it is to use the scanned image.
If you really want to use the scanned image, though, you can. Remember that
the image is just that, a picture, not text. You can put that picture in
your header or footer and set the picture properties to be behind text
without wrapping.
Then, in the body of your document, you construct an online form using the
forms toolbar. You position the fields using either a table or frames. For

You could also use OCR software that came with your scanner to get it turned
into text that could be edited. It might be easier to retype it though.
