Jeff,
"AUTOTKIT.EXE" from HP is known to cause Toolbar resets.
Also several other 3rd party (adware\parasite) BHO\Toolbars
can cause resets ....

and then lock the toolbars (which, by the way, isn't necessary to save their
positions). When I then log off and back on, the toolbars are back in their
