CCleaner has more extensive cleaning capabilities, but don't use the
registry cleaner. Just use it to remove unneeded files.
Defragmenting a disk takes time. There are 3rd party tools, but frankly you
don't need them. That said there are several good ones including
PerfectDisk andDiskeeper. I'm not personally familiar with any others.

Diskeeperproffesional has a screen saver mode where it will defragment
whenever you are not using your PC.
Having said that there is usually no reason you can't run Windows
defragmenter and still use your PC.
It's not like using Win98 where you couldn't use your PC. The only problem I
have had doing this is video files stuttering when they are actually being
moved by during the defragmenting process, otherwise I doubt you would
notice, though I suppose it depends on the speed of your PC.
