removing start-up programs

I'm referring to "msconfig" I know how to stop the program from opening by
unchecking it but how can I remove it from the list which keeps getting
longer and longer.

I'd just like to have listed the current programs/files that will actually
open.

I run XPSP2 prof. with all the updates in place.

I use Msconfig Cleanup when a bad program left it's entry even after
uninstalling it
