In general, how often do people reformat given the fact that there's now so
much bad stuff floating around?

If you have a good firewall and a good virus scanner, and if you
stay away from suspicious Internet sites, and if don't fall for these
enticing offers ("Speed up your PC", "Clean your Registry",
"Double your download speed", "Your PC is vulnerable"), and
if you don't run all these fun programs that well-meaning friends
send you by EMail then you can run a Windows installation
for several years.

Good question. I do a clean (re) install every 9-12 months. And evry
time I'm staggered by how much space I "reclaim" from before the
install to the time all my programs and data is recovered. Staggering,
despite running all sorts of so called cleanup and cleanout software
on a regular basis.

On my own systems I never reformat and reinstall unless I have a major
reason to do so such as system failure or I've contracted a really nasty
virus that would take me longer to clean out than start a fresh.
 
I much prefer to image my drive and then, should a problem occur, reimage it
back to the original drive/partition. It is much less work than
re-installing. However, sometimes, even for me, it becomes necessary to
reformat, but i think the last reformat was 18 months or so ago.

Only after a hard drive failure makes it pretty much the only option.

More often than they need to. But it's sometimes easier for people who
don't have anything they really want to save, or who have all their
stuff backed up.
